Three Different Application Forms

There are application forms below for the following:

Reader's Program Form
Diaconal Program Form
General Application Form

Those applying for the Reader's Program or Diaconal Program fill out only the form for that program.

Those who are not studying officially for the diaconate but who want to take individual courses or all of the courses in the diaconal program, should fill out the General Application Form. 

Application Deadlines

There are currently no registration deadlines. Normally, courses will begin shortly after applications and fees have been received and processed. Both parts of the Reader’s Program and the 20 courses of the Diaconal Program are offered by correspondence. Courses may be applied for at any time during the year, subject to availability of the teacher at the time.

Application Forms

	You can download a copy of a form by right clicking your mouse. Right click on the format you require and choose either "Save Target As..." (Internet Explorer) or "Save Link As..." (Firefox, Netscape). You will then be given the option to save a copy of the file.
